1. First-Time Social Insurance Application for Enterprises
Pursuant to the process of collecting social insurance, health insurance, unemployment insurance, occupational accident insurance, and occupational disease insurance contributions, and managing social insurance books and health insurance cards issued together with Official Letter No. 2089/VBHN-BHXH dated June 26, 2020 of Vietnam Social Security:
Article 23. Registration and adjustment of contributions to social insurance, health insurance, unemployment insurance, occupational accident insurance, and occupational disease insurance; issuance of social insurance books and health insurance cards.
1. Required Documents
1.1. Employees
a) For employees working at the company:
– Declaration for participation in and adjustment of social insurance and health insurance information (Form TK1-TS).
– If the employee is entitled to higher health insurance benefits, supporting documents must be added (if any) in accordance with Appendix 03.
b) For employees working abroad under contracts referred to in Sections a, c, and d, Point 1.7, Clause 1, Article 4:

– The overseas labor contract with a defined term, or the extended labor contract together with the written extension agreement, or the new labor contract signed in the host country under the original agreement.
1.2. Employer
a) Declaration of the employer’s participation in and adjustment of social insurance and health insurance information (Form TK3-TS). b) List of employees participating in social insurance, health insurance, unemployment insurance, occupational accident insurance, and occupational disease insurance (Form D02-TS).
c) Information statement (Form D01-TS).
2. Number of Application Sets: 01 set.
* For companies that register late after the issuance of their business license and are subject to retroactive social insurance and health insurance contributions, the additional documents are:
– Written explanation from the employer (Form D01b-TS);
– Payroll and remuneration statement of the employer (copy); labor contracts (copy, 01 per employee) corresponding to the retroactive contribution period.
3. Place of Submission: District-Level Social Security Office (register with the social security agency in the locality stated on the business registration license)
– Employers headquartered in any province must register for social insurance in that province in accordance with the jurisdictional decentralization of provincial social security agencies.
– Branches of enterprises pay social insurance in the area where their branch business license was issued.
4. Submission Methods:
Submit dossiers and receive results of administrative procedures through electronic transactions or postal services, specifically:
– For paper dossiers: Enterprises notify the social security agency or postal service provider to collect dossiers directly at the company’s premises. Electronic data (if any) is sent to the social security agency via the internet.
– If the employer registers for electronic transactions, it must comply with the regulations on electronic transactions in carrying out procedures for participation in social insurance, health insurance, and unemployment insurance, and issuance of social insurance books and health insurance cards of Vietnam Social Security.
2. First-Time Social Insurance Registration Dossier for Employees
The required documents differ depending on the employee’s situation. Specifically:
For employees working at an employer, the dossier includes:
1. Declaration providing and updating information of participants in social insurance and health insurance (Form TK1-TS).
The employee submits the application to the employer. The employer enters the social insurance number on the corresponding forms for employees who have already been issued a social insurance code, and assists employees who have not yet been issued a code (including those who do not remember their code) in preparing the Declaration of participation in and adjustment of social insurance and health insurance information (Form TK1-TS), before submitting it to the social security agency.
For employees working abroad, the dossier includes:
- Declaration of participation in and adjustment of social insurance and health insurance information (Form TK1-TS);
- An overseas labor contract, or an extended labor contract together with the written extension agreement, or a new labor contract signed in the host country under the original agreement.
The dossier is submitted through the unit dispatching the employee to work abroad, or paid directly to the social security agency in the employee’s place of residence before departure.
If the employee participates in voluntary social insurance, the dossier includes:
- Declaration of participation in and adjustment of social insurance and health insurance information (Form TK1-TS)
Employees submitting directly to the social security agency must do so at the One-Stop Department of the social security agency or at a Public Administration Service Center at any level.
If the employee submits through a collection agent, the agent shall prepare a list of voluntary social insurance participants (Form D05-TS) and submit it together with the participant’s Form TK1-TS declaration to the social security agency.
